Kentucky charges heavy trucks a weight-distance tax called KYU. If you run on Kentucky roads with a combined license weight greater than 59,999 lbs, you need a KYU license (KYU number) and you file quarterly returns. [1]

Who needs a KYU number
KYU is a tax license for all carriers traveling on Kentucky roadways with a combined license weight greater than 59,999 lbs, that is, 60,000 lbs and up. [1]
How to apply
Apply online. First-time users must set up a KYID account before applying. The KYU number is issued when you complete the online application. If the online application has technical problems, you can mail Form TC 95-1, the Kentucky Trucking Application, which takes 10 to 14 business days to process.
If you don't make regular trips through Kentucky, you can buy a one-time temporary KYU permit per truck instead. [1]

Under KRS 138.660, the KYU tax is 2.85 cents ($0.0285) per mile. For example, 1,000 miles on Kentucky roads comes to $28.50. [1]
Quarterly returns, including zero-mile returns
KYU returns are filed quarterly online for all qualified vehicles. If you didn't travel in Kentucky during a quarter, you must still file zero miles, or you face a penalty, interest and a $500 revocation fee. [1]
